About the Program 

We are Victoria’s largest regional health service with one of the busiest hospitals (The University Hospital Geelong) in the state. We serve the Barwon South West region with over 500,000 people through the efforts of over 6,500 staff. We provide care at all stages or life and circumstance through a wide range of services from emergency and acute to mental health, primary care, community services, aged care, and subacute/rehabilitation. Care is provided to the community through many key locations throughout the region.

The Transition Care Program provides short term care for clients in their homes and in residential aged care settings. Transition Care provides goal oriented, time limited and therapy focused care to help older people at the conclusion of a hospital stay.

About the role 

An exciting clinical leadership opportunity in the Barwon Health Transition Care Home Based Program team to provide clinical leadership and service delivery for TCP clients in their homes.

We are looking for an organised, dynamic and flexible team player with extensive clinical knowledge, skills and experience in Gerontology Physiotherapy in a wide range of clinical areas. As a senior member of the team, you will provide leadership to ensure best practice Physiotherapy service delivery, and responsibilities will include the support of professional development, clinical supervision and quality improvement of Grade 2 Physiotherapists and Allied Health Assistants.

The clinical component of the role will include the further development and delivery of Physiotherapy services to the Home Based Program and support Grade 2 Physiotherapists in the Residential Transition Care Program located at Alan David Lodge in Charlemont.  As part of a dynamic Multi-Disciplinary Team you will provide high quality assessment and treatment for Transition Care Program clients in the community.
