About Barwon Health
Barwon Health is the major regional health provider for the Barwon South West region. It is one of Victoria’s largest regional health service with one of the busiest hospitals in the state, University Hospital. We serve over 500,000 people through the efforts of over 8,000 staff and more than 1200 volunteers. We provide care at all stages or life and circumstance through a wide range of services from emergency and acute to mental health, primary care, community services, aged care, and subacute/rehabilitation. Care is provided to the community through over 28 key locations throughout the region.
About the role
As the Registered Nurse (RN) you will be responsible for delivering nursing care within an outpatient cardiac rehabilitation program. This role is focussed on improving the health and wellbeing of people with a range of cardiac conditions who require short-term, individualised rehabilitation that is delivered in a group setting.
You will have a key role in clinical assessment, the provision of client education, monitoring of recovery, discharge planning and client follow-up and review.
You will be a key member of the multidisciplinary team who works collaboratively to ensure that all assessment, care planning, intervention and discharge planning is client-centred and driven by the best available evidence.
You will apply clinical nursing skills required for client care that are within scope of practice and appropriate for the setting and the rehabilitation program.
