Co-located with other Chronic and Complex disease teams.  The working environment is friendly and supportive.   

Being situated in suburban location allows for easier parking opportunities.

Outdoor sitting area.  Shower facilities on site. 

About the role 

This role will be monitoring COVID +ve clients utilising remote monitoring technology.   

The Remote Patient Monitoring team service clients in the community  with Chronic Disease particularly heart failure, COVID and others such as diabetes and COPD.   Remote Patient Monitoring is a term commonly used to describe the use of technology to support patients in their home environment by transmitting their data back to a clinical base where the nurse clinician can review the data and support the patient as required. 

Weekend and on call are a part of the requirements of this position.

RPM is small  enthusiastic team who work cohesively to provide the highest quality care to our clients in the community.   The service is centred on the patient and is guided by Barwon Health values. 

The team have worked with many redeployed staff over the past months to support the COVID monitoring program.   The expanded team have a 'can-do' attitude, with a sense of pride in the service they are providing to the community due to the pandemic.
