About the facility
Colac CMHT is part of the wider Barwon Health MHDAS continuing care service. We function as an integrated service, providing crisis and scheduled intake assessments, drug and alcohol intake services and case management within an experienced and supportive team.
About the role
As the successful applicant, you will provide assessment, treatment and recovery based interventions to individuals and their families who experience serious mental illness. The broader service delivery model of MHDAS includes provision of high quality interventions that are consumer-centred, family/carer inclusive and are delivered in a timely and coordinated manner in line with the Victorian Mental Health Act 2014. Other duties include:
- Day-to-day liaison with consumers, families and other services regarding care and treatment planning within a recovery framework and evidenced-based interventions
- Day-to-day clinical decision making including assessment, treatment, risk management strategies and prioritisation
- Participation and facilitation of peer case reviews within the multidisciplinary team
