About the facility
Barwon Health Palliative Care provides palliative care nursing, medical, allied health and volunteer services within a framework of integrated health service integrating community, inpatient as well as an acute consultative service. The model of palliative care is based on level of need and targets patients with complex care needs.
Community Palliative Care: provides specialist care in a client’s home or outpatient clinic as appropriate. Our aim is to ensure that the most appropriate mix of services and professionals are available to provide care and support to patients and their family/carers. Community Palliative Care Specialist staff provide:
- Assessment for symptom management
- Advice and support utilising a needs based model of care
About the role
An exciting opportunity is available within Barwon Health for an enthusiastic Community Palliative Care Nurse. The position will be based within the Community Palliative Care program at the McKellar Centre, North Geelong.
You must be registered with APHRA, along with post-graduate qualifications in palliative care or oncology nursing. You will have excellent communication skills, be well organised in your work practice and an enthusiastic team player.
