About the role

Building on the success of other bed substitution models we propose to provide an equivalent level of inpatient palliative care within a person’s home. The Service Improvement Lead will work with the Community Health, Rehabilitation and Palliative Care directorate to undertake a strategic project within the service offerings of the directorate.  

The position requires collaborative work with managers, clinicians and broader stakeholders to support planning and implementation of these new initiatives.
