About the facility 

Barwon Health's Connected Care Program and Hospital Admission Risk Program (HARP), are community based services employing nurses, allied health (incl. social worker, physiotherapists, exercise physiologists, dietitians, occupational therapy, psychology, neuropsychology) and medical staff that provide assertive outreach and clinic based support for people with chronic health and/or complex psychosocial needs. Both the Connected Care Program and HARP aim to decrease hospital demand through the provision of comprehensive assessment, care planning and care coordination, and addition to supporting timely access to specialist care, for people who are frequently, or are at imminent risk of present to hospital.

About the role 

The Barwon Connected Care Program and HARP are looking for an experienced allied health professionals (or nurse) to join their team. The position will be located at either the Northern Suburbs, and/or at Belmont, Geelong, which can be discussed at interview.

As a successful candidate you will provide comprehensive assessment, care planning and care coordination for a case load of clients living within the Barwon Health catchment. You will be expected to work collaboratively with the client and their family, your colleagues, in addition to internal and external service providers, to work towards improved client health, wellbeing outcomes and self-management.
