About the facility 

Barwon Health's Advance Care Planning (ACP) program consists of an active, enthusiastic, flexible and innovative team with a focus on person centred care, shared decision making, safety and quality improvement. The program has received a number of quality awards for innovative projects which improve the overall service to our community. We have had opportunities to present our work at local, national and international fora.

About the role 

As the ACP Clinician you will work closely with the program manager and team for the ongoing implementation and facilitation of ACP. The ACP Clinician is expected to facilitate (ACP), utilising a shared decision-making process with referred patients from all health directorates, with a focus on patients who are at risk of clinical deterioration, have chronic or life limiting illness or are aged and frail. You will assist individuals to participate in the ACP process including documentation as required, developing Advance Care Directives, identifying or appointment of Medical Treatment Decision-Makers and Goals of Management/Care forms.

The role may also include education, training and mentoring of staff across Barwon Health directorates and sites and external organisations within the region. You will be innovative and flexible in undertaking the role and will be expected to participate in quality improvement initiatives and project work, in consultation with the Manager.

Essential: Registered Nurse (YT12), current registration with Australian Health Practitioners Regulation Agency
Desirable: Advance care planning training and experience
Essential: Substantial clinical experience in the areas of acute, sub-acute, residential aged care and/or community care
Essential: Experience in providing patient care in situations where advance care planning is applicable
Essential: Highly developed communication and interpersonal skills
Essential: Commitment to flexibility and innovation in practice including an evidence-based approach to care
Essential: Demonstrated commitment to professional development
Desirable: Ability to work effectively with internal and external stakeholders
Desirable: Project work or change implementation experience
