About the facility
Barwon health is the major regional health provider for the Barwon South West region. It is Victoria’s largest regional health service with one of the busiest hospitals in the state, University Hospital. We serve over 500,000 people through the efforts of over 7,000 staff and more than 1000 volunteers. We provide care at all stages or life and circumstance through a wide range of services from emergency and acute to mental health, primary care, community services, aged care, and subacute/rehabilitation. Care is provided to the community through over 21 key locations throughout the region.
About the role
The Regional Wound CNC role contributes and is a part of the community - based, multidisciplinary area health team to support clients and carers to achieve an optimum level of health independence through:
- Comprehensive, co-ordinated and integrated multidisciplinary services,
- Provision of client –centred care to community members from a broad range of backgrounds.
- Contribute to the development and delivery of high quality, cost effective community-based wound management services.
- Aim to improve outcomes for clients of Home and Community Care Program for Younger People (HACC PYP) and Commonwealth Home Support Program (CHSP) funded district nursing services and high care residents of public sector funded residential aged care facilities (PSRACS) of the Barwon region of Barwon-South West Region of Victoria, by strengthening the wound management practice of staff providing these services.
- Through clinical demonstration and secondary consultation, the Wound Management Clinical Nurse Consultant will build capacity in staff through mentoring, onsite support and the delivery of specialist training sessions.
