About the facility 

Belmont Primary Care is located at the redeveloped Belmont Community Health Centre. The centre is shared with numerous teams including community rehabilitation, community nursing, and immunisation, dental and mental health. It is a fabulous opportunity to work closely with these other co-located teams.

About the role

An exciting opportunity exists to be part of the community-based, multidisciplinary team to support clients and carers to achieve an optimum level of health independence through:

• Supporting self-management and referral pathways for clients with complex health and psychosocial needs to enable them to move towards positive health behaviours and actively manage their health care needs
• Collaboration and building sustainable relationships with other programs within Barwon Health and the broader community.
• Proactively exploring ways to increase the referral base through targeting areas of need in the local community and developing strategies to reach these clients.

All activities of the position will reflect current best practice standards for Community Health Nursing and clinical portfolio responsibility. It will be located at a primary location but may be moved to other Barwon Health locations depending on service delivery needs.
