About the facility 

Stroke Association Victoria and Barwon Health Stroke Service are collaborating to support stroke survivors. The dedicated stroke service has been established across Barwon Health for more than 15 years. Multidisciplinary, clinical teams are committed to the provision of best care and outcomes for stroke survivors from Geelong and the Barwon Southwest region focussing on: Engaging in a collaborative journey

About the role 

To work within the Barwon Health Stroke Service contributing to the Stroke Association Victoria Community Hub, ‘Stay at Work Initiative Project’. This temporary part time role will require identifying stroke survivors who would benefit from the project services through both a prescribed screening process and liaison with Barwon Health stroke multidisciplinary teams. 

The position will successfully develop screening tools and establish a communication network that effectively connects identified stroke survivors to the SAV support network. Contribution to Project documents and data collection is essential.

About the culture

A well-established stroke unit with a strong MDT that has successfully lead stroke practice in Barwon Southwest region since 2000, aspiring to the Clinical Guidelines for Stroke Management and Stroke Clinical Standard through clinical practice and support data collection.
