About the facility 

Hospital Admission Risk Program (HARP) is a community-based program aimed at decreasing hospital demand through the provision of comprehensive assessment, care coordination and timely responsive specialist care in the community setting, for people with chronic health and complex psychosocial needs.

About the role 

As a HARP Team Leader, you will support the day to day human resource management and clinical supervision of HARP Community Clinicians, in addition to supporting the HARP Coordinator and Manager of Chronic and Complex Care with quality improvement initiatives.

The HARP Team Leader is a member of the HARP Leadership Team, which is responsible for the planning, service delivery and clinical support of HARP. All HARP Team Leaders have a client case load/care coordination element to their role.
