About the facility

The Transition Service provides comprehensive assessment for young people (16-25) with complex health/disability issues in order for them to develop a Care Plan that will detail the requirements for a successful transition journey from the paediatric health sector into the adult health sector.  Intervention focuses on achieving transition goals and acquiring skills of adult independence. Promotion and secondary consultation ensures young people (<16) are prepared, as they grow up, to make their health transition a successful one.

Located at Barwon Health's McKellar site, the Transition Service enjoys the benefits of being co-located with the Victorian Paediatric Rehabilitation Service (VPRS) and with other key Community Health and Rehabilitation Services.

The Transition Clinic located at the McKellar Centre are Seeking an Occupational Therapist – Grade 2 to join their friendly and supportive team.

As the Occupational Therapist you will provide specialist assessment and intervention for young adults with complex disability transitioning from paediatric to adult health services. You will ensure a best practice and evidenced based approach to the delivery of services.

This position is based at the McKellar Centre, North Geelong. There may be some travel required to other Health Services, local service providers, community organisations and schools.

Selection Criteria Desirable: Bachelor of Applied Science (Occupational Therapy) or recognised equivalent. Registered as an Occupational Therapist with the AHPRA.
Desirable: Current Victorian Driver’s Licence
Desirable: Working with Children’s Check
Desirable: Highly developed conceptual and analytical skills and the expertise to resolve complex problems.
Desirable: Ability to communicate effectively with a wide range of people, including a diverse client base, community service agencies, community groups and individuals who have limited resources
Desirable: Ability to provide effective professional leadership, mentoring and clinical education
Desirable: Skill in the evaluation and implementation of evidence based practice and modification of service delivery
Desirable: Developing and presenting education programs
Desirable: Experience working with young adults with complex disability.
Desirable: An understanding of the LIFEspan Service (Living Independently and Fully Engaged) Model of Transition.
Desirable: Experience in working in a co-ordinated, co-located rehabilitation team structure
