About the role 

This position requires the successful applicant to deliver direct client care in the home while ensuring the clients broader health and well-being issues are supported. Community Nursing care is delivered in partnership with the client/family through the development of goal directed planning.

This position requires the successful applicant to work a minimum of 40hrs a fortnight with the flexibility to work weekday, weekend and afternoon shifts. The successful applicant will bring with them 'current' Community Nursing skills and experience, along with confidence in using clinical information systems and a strong commitment to working with a professional team.
