About the facility
Barwon Health provides a Hospital in the Home service that has been operating for over 18 years. The program provides care in a patient's home that would otherwise be provided in an acute hospital.
About the role
In this role you will be ward (inpatient and ED) based and will be responsible for the identification and facilitation of patients who will be discharged from the acute setting to another bed based service or to the community with appropriate supports in place. As the successful applicant you will have:
- Current registered nurse, Division 1, or Senior Allied Health clinician with AHPRA
- Significant post registration experience
- Extensive clinical knowledge, skills and experience
- The ability to work autonomously, meet deadlines and contribute effectively as a team member within a multidisciplinary environment.
- Excellent organisational skills and the ability to prioritise and manage multiple tasks.
- Sound knowledge of the high volume discharge pathways
