About the facility
Barwon Health is Victoria's largest regional health service. We provide high quality, safe, people-focused care to more than 450,000 people in Geelong and South Western Victoria.
Newcomb & Corio Community Nursing is located at the Newcomb & Corio Community Health Centres. The centres are shared with numerous teams including primary care (allied health and community health nursing), dental, carer support services, dialysis and mental health services. Providing a fabulous opportunity to work closely with these co-located community health teams.
About the role
The role sits within the community nursing team and will contribute as part of the community based multidisciplinary area health team to support clients and carers in their home to achieve an optimum level of health independence through:
- Close collaboration with community nurses through joint assessment, consultation and education
- Provision of responsive community OT interventions in the home, with a broad range of client diagnoses and complexity
- Provision of client centred care in line with the active service model principles
- Contribute to the development and delivery of high quality, cost effective, community based occupational therapy services
- Contribute to meeting the program and area health team key performance indicators (KPI’s)
