About the facility 

Barwon Health is one of the largest and most comprehensive regional health services in Australia, providing care at all stages of life and circumstance. Health services available through Barwon Health cover the full spectrum from primary care, community services, aged care, rehabilitation, mental health, emergency and acute care. The Barwon Health Hospital in the Home (HITH) program is a well-established service currently providing home based acute health care and home based cancer care to over 2000 patients each year. 

About the role 

The Hospital in the Home Clinical Nurse is responsible for the clinical care co-ordination and delivery of clinical care to patients referred to and on the Hospital in the Home program.

Your role will be to provide patient focused holistic nursing care to those assigned to you, in order to achieve desired clinical outcomes as well as liaise with community nursing staff providing support and direction in relation to clinical care of HITH patients. Experience and a post graduate qualification in oncology will be well regarded. The ability to work autonomously, meet deadlines and work effectively as a team member within a multidisciplinary environment is important within this role.
