About the facility 

HARP is a community-based program employing more than 30 staff, including nurses (Div 1 & Div 3), physiotherapists, social workers, occupational therapists, dietitians and administration staff that provide outreach support for people with complex health and psychosocial needs. HARP aims to decrease hospital demand through the provision of comprehensive assessment, care coordination and timely access to specialist care in the community setting for people with complex health and/or psychosocial needs, who present frequently or are at imminent risk of presenting to hospital.

About the role 

The Hospital Admission Risk Program are looking for an experienced Administration team member to support a large multidisciplinary team and deliver consumer focused administrative support and to assist the HARP team by providing efficient and effective clerical support so as to achieve desired outcomes.

The successful candidate will provide clerical and reception duties with attention to detail, and in accordance with Barwon Health policy and procedures. They will be required to and assist the Coordinator and Team Leaders to support maintaining an efficient work place and service delivery.

The HARP Administration position is often the first contact for consumers, internal and external service providers and community members either by phone or email. The successful candidate is required to use the AIDET framework in practice, provide an efficient point of contact for HARP staff, and accurately document required information. They are also responsible for processing all new HARP referrals, and discharges, and contribute to quality improvement.

The successful candidate will be required to work 30 hours per fortnight, 7.5hr days of a Wednesday and Thursday.
