About the facility 

Formed in 1998, Barwon Health is one of the largest and most comprehensive regional health services in Australia, providing care at all stages of life and circumstance. Health services available through Barwon Health cover the full spectrum from primary care, community services, aged care, rehabilitation, mental health, emergency and acute care. The Barwon Health Hospital in the Home (HITH) program is a well-established service currently providing home based acute health care to over 2000 patients each year. 

About the role 

In this maternity leave relief role you will be responsible for the clinical care co-ordination of patients referred to and on the program. Your role will be to provide patient focused holistic nursing care to those assigned to you, in order to achieve desired clinical outcomes as well as liaise with community nursing staff providing support and direction in relation to clinical care of HITH patients. Experience and a post graduate qualification in oncology will be well regarded. The ability to work autonomously, meet deadlines and work effectively as a team member within a multidisciplinary environment is important within this role.

Desirable: Post graduate qualifications in discipline

Desirable: The ability to work autonomously, meet deadlines and work effectively as a team member within a multidisciplinary environment
