As the Community Nurse you will be responsible for the provision of patient focused holistic nursing care to those assigned to you in order to achieve desired clinical outcomes. You will have highly developed interpersonal skills as well as sound communication skills both verbal and written.

As the successful applicant you will need to have current/recent acute or community health experience. Experience & knowledge in the areas of wound care, palliative care, community health care principles (Active Service Model & Social Model of Health) and continence management is desirable.

Qualifications, Experience and/or Specialist Knowledge –

ESSENTIAL:

• Degree level qualification with a recognised educational institution in Nursing.
• Registration with AHPRA as a Registered Nurse Division 1
• Current Victorian Drivers Licence
• Information Technology skills including MS Office, Outlook and patient management systems.
