About the facility 

Belmont Community Rehabilitation Centre services are time limited and delivered according to a Rehabilitation plan that is based on goals negotiated with the client and their careers. Our state of the art facilities are located on site with Belmont Primary Care, Community Nursing, Dental, Mental Health and Immunisation services.

About the role 

The Orthopaedic Rehabilitation Care Coordinator is based at Belmont Community Rehabilitation Centre and has a key role in supporting the client journey and delivering Barwon Health’s R3 strategy, this includes: 

  • Ensuring the team supports and optimises patient flow from bed based services; Intake, triage and prioritising new referrals;
  • Ensuring clients receive comprehensive assessment;
  • Ensuring a consumer centred goal-directed model of care throughout the client journey and that this is reflected by the clients care plan;
  • Leading service development activities within the team; and actively seeking education and training opportunities that support the rehabilitation team, service model and Barwon Health orthopaedic service development objectives.
  • To provide clinical supervision, support and development of physiotherapists within the orthopaedic stream at BCRC.

Essential: • Degree in Physiotherapy from an Australian Physiotherapy Council accredited course.
Essential: Current national registration as a Physiotherapist with the Australian Health Professionals Regulation Agency.
Essential: Eligible for full membership of the Australian Physiotherapy Association.
Essential: Current Victorian Motor Vehicle Driver’s License.
Essential: Extensive clinical knowledge and experience in orthopaedic Physiotherapy.
Essential: Experience working in a sub-acute rehabilitation setting.
Essential: Demonstrated ability to work collaboratively in an inter-disciplinary team with a clear consumer focus.
Essential: Proficient in the use of information technology.
Desirable: Post-graduate qualifications in a relevant clinical area.
Desirable: Experience in clinical research.
Desirable: Current membership with the Australian Physiotherapy Association.
