About the facility
Barwon health is the major regional health provider for the Barwon South West region. It is Victoria’s largest regional health service with one of the busiest hospitals in the state, University Hospital. We serve over 500,000 people through the efforts of over 7,000 staff and more than 1000 volunteers. We provide care at all stages or life and circumstance through a wide range of services from emergency and acute to mental health, primary care, community services, aged care, and subacute/rehabilitation. Care is provided to the community through over 23 key locations throughout the region.
The team leader role is currently situated at 120 Settlement Rd, Belmont, Geelong.
About the role
The Social Supports Program (SSP) is described as ‘an opportunity for clients to attend and participate in social interactions which are conducted away from the client’s home’ through structured activities that ‘are provided in a group-based environment and designed to develop, maintain and support social interaction and independent living.’
The SSP team leader is a part of a broader team working with the individual participants and other staff to optimise the health and independence of clients. They play a key role in monitoring the person’s progress towards their goals and ensuring that the program structure meets the needs of the client.
