About the Facility
The Neurology Rehabilitation Care Coordinator ensures the provision of a consumer centred and goal directed model of care within the Neurology Rehabilitation Team. This team provides outpatient (home, centre and community based) rehabilitation programs to people who experience a wide range of neurological conditions. This care is delivered through a coordinated team of medical and allied health clinicians.
About the role
The Neurology Rehabilitation Care Coordinator is based at the McKellar Community Rehabilitation Centre and has a key role in supporting the client journey.
This includes:
- Ensuring the team supports and optimises patient flow from bed based services
- Intake, screening and prioritising new referrals
- Ensuring clients (where appropriate) have access to interdisciplinary comprehensive assessments;
- Ensuring a consumer centred goal-directed model of care throughout the client journey and that this is reflected by the clients care plan
- Leading service development activities within the team
- Actively seeking education and training opportunities that support the rehabilitation team and service model.
