Position Summary
  • The role will be responsible for the analysis of data pertaining to future planning for service delivery in the community setting
  • Collaboration with clinical teams and understanding of patient data sets will be developed and valued by the broader organisation
  • Make a significant contribution towards providing service for some of the most vulnerable patients in the Barwon Region

About the facility

The Healthy@Home team are newly established to provide community based care to a defined patient group who are identified at high risk of representation to hospital.

The Healthy@Home team work closely with HARP and other hospital avoidance programs to establish the best long and short term supports for patients in the community.

About the role

The Healthy@Home Data project role will use data to evaluate the effectiveness of current clinical strategies and inform future planning for HealthLinks interventions in collaboration with the Service Improvement Project Lead. The data project role will also inform the future of data collection for the HelathLinks initiative to meet reporting requirements.

Essential: Tertiary qualifications in Health Information Management, Business, Information Technology or related discipline.
Essential: Advanced level knowledge of Microsoft Excel

Essential: Familiarity with health care systems including iPM, TCM, BOSSnet and Symphony.
