Barwon Health's Advance Care Planning program consists of an active, enthusiastic, flexible and innovative team with a focus on person centred care, shared decision making, safety and quality improvement. The program has received a number of quality awards for innovative projects which improve the overall service to our community. About the role

The Advance Care Planning (ACP) Clinician role is to work closely with the ACP program manager for the ongoing implementation and facilitation of ACP.

The ACP Clinician is expected to facilitate (ACP), utilising a shared decision making process, with referred patients from all health directorates with a focus on patients who are at risk of clinical deterioration, have chronic or life limiting illness and those aged over 75years.

The ACP Clinician will assist individuals to participate in the ACP process including documentation as required.

The role may also involve education, training and mentoring staff across Barwon Health directorates and sites and external organisations within the region.

The ACP Clinician will be innovative and flexible in undertaking the role and will be expected to participate in quality improvement initiatives and project work, in consultation with the ACP Manager.
