About the facility
Rehab at Home, based at the McKellar Centre, will provide Rehabilitation to people living in their own home and is part of the state wide Better@Home initiative.
An exciting opportunity exists to join the new Rehab at Home team. The successful applicant will be an experienced Registered Nurse with a passion for rehabilitation and working in an interdisciplinary team model. This is an exciting new program that will allow qualified applicants to start at the commencement of this innovative service.
About the role
As the Rehab at Home Grade 3 Registered Nurse you will:
- Provide high quality care to patients in the Rehab at Home program, and to provide professional leadership within their clinical speciality and Barwon Health.
- Contribute to professional issues within the organisation to ensure the promotion of nursing professional values and high standards of nursing service provision.
- Work in collaboration with the Program Coordinator and all other stakeholders to monitor, develop and improve nursing clinical service provision consistent with the objectives of the Barwon Health and the CHRPC Strategic plans.
Parameters of this position would include independent delivery of skilled nursing care to Rehab at Home patients and significant opportunity for service assessment and improvement.
