About the facility

The Hospital Admission Risk Program (HARP) is a community based multidisciplinary program, including nurses, physiotherapists, occupational therapists, dietitians, psychologists and administration that provide outreach support for people with chronic health and psychosocial needs, HARP aims to decrease hospital demand through the provision of comprehensive assessment, care planning, care coordination and facilitating timely access to specialist care, for people who present frequently or are assessed as being at imminent risk of presenting to hospital. 

About the role

As a HARP Team Leader you will provide clinical leadership and support to a group of HARP Community Clinicians, in addition to line management, coordinating data to day workload, leave management, professional development reviews, accountability sessions and participate in and lead quality improvement initiatives. HARP Team Leaders will also be involved in support the HARP and broader Chronic and Complex Program navigate through change.

The clinical component of the role is provision of direct client care to HARP clients.
