About the facility 

Belmont Primary Care is located at the recently redeveloped Belmont Community Health Centre. The centre is shared with numerous teams including community rehabilitation, community nursing, and immunisation, dental and mental health. It is a fabulous opportunity to work closely with these other co-located teams.

About the role 

An exciting opportunity exists to contribute to and be part of the community - based, multidisciplinary area health team to support clients and carers to achieve an optimum level of health independence through:

  • Provision of diabetes education and self-management skills to clients diagnosed with all types of diabetes including type 1, type 2, gestational and pre diabetes. This includes individual assessment, education and shared care planning and the provision of ongoing support and advocacy
  • Comprehensive , co-ordinated and integrated multidisciplinary services
  • Provision of client –centred care to community members from a broad range of backgrounds.
  • Contribute to the development and delivery of high quality, cost effective community-based diabetes Education services.
  • Contribute to meeting the program and area health team key performance indicators (KPI’s)

It will be located at a primary location but may be moved to other Barwon Health locations depending on service delivery needs.

Selection Criteria Essential: A qualification in or working towards Credentialed Diabetes Education as defined by the ADEA.
Essential: Current full membership of ADEA.
Essential: A current Victorian Driver’s Licence
Essential: Ability to work effectively as a member of a multidisciplinary team and contribute to a positive team environment
