About the facility
The Surfcoast Primary Care team operates from the Anglesea and Torquay Community Health Centres and works in close partnership with stakeholders across the Surfcoast region. This role offers the opportunity to support the delivery of innovative and evidenced-based care across a growth corridor.
The Primary Care team incorporates multidisciplinary health professionals from allied health and nursing backgrounds. This dynamic and high performing team provides services ranging from early intervention through to the support of consumers with chronic and complex needs
About the role
The primary objective is to coordinate daily operations of the local Primary Care team and assist in planning, implementation and organisation of clinical care. The role requires working in close partnership with key stakeholders to ensure successful integration of high quality clinical services. The successful candidate will be driven to ensure that a person-centred approach is embedded to achieve optimal health outcomes. Demonstrated accountability, effective communication and time-management skills are essential. The successful candidate will also be able to demonstrate the ability to lead and motivate staff through change.
