Position Summary

Barwon Health’s Mental Health Drugs and Alcohol Service is going through a period of rapid growth and expansion and requires additional staff to support this growth. This is an exciting chance to implement a new program within our community services.

  • Be part of the implementation process of a new program-Community Intensive Treatment Team (CITT)

About the service

Community Intensive Treatment Team  (CITT) is located in the Torquay Community Health Centre (CHC), and sits alongside the Surf Coast CCS where standard case management is provided.

CITT is a community program, where clinicians are required to provide assertive follow up and linkages to consumers experiencing high complex needs.

Clinicians are required to be flexible and work a rotating roster, however the main shift is Mon-Fri, 0830hrs-1700hrs

About the role

CITT focuses on providing intensive care and treatment for the most severely ill group of individuals with mental illness in the community. In particular, those that are hard to engage, at risk of hospitalisation, and those that require high intensity psychosocial input. CITT aims to ensure continuity of care, to prevent admissions, and to stimulate inclusion, so that individuals can participate in society.
