About the department
Barwon Health Carer Support is a multidisciplinary team of qualified clinicians located both in Geelong and Warrnambool. Barwon Health Carer Support receives both state and federal funding to provide flexible and meaningful support options to the many unpaid family carers across the Barwon South West region of Victoria. We are also the Barwon South West outlet for the national Carer Gateway program.
About the role
Working within a multi-disciplinary team environment (telehealth / face to face) this position focuses on the provision of government funded carer support services that are responsive to the needs of the local community. This position encompasses a variety of roles including information provision; guided referrals; facilitation of peer support groups; community outreach; developing carer capacity and resilience; delivering practical and innovative practical support.
In this role you will:
- Undertake holistic assessments of clients' need and develop goal-directed care plans
- Respond to emergency, practical and emotional needs of carers
- Coordinate service delivery and refer to other service types as required
- Facilitate peer support groups
- Contribute skills, knowledge and expertise to promote community awareness and understanding of carer issues though outreach and promotional activities
